Updated February 19, the latest Point of Contact list for our Care Coordination team is here. Please contact any of the names, numbers on this contact sheet to link with a Care Coordinator you do not have the contact information for.  Anyone in any region will be glad to help you!


Did you know the Care Coordinator's Supervisor is listed on the Life Plan?  The auditor listed on the first page of the Life Plan is the Care Coordinator's supervisor, so if you are looking to get in touch with the supervisor you can use this information. 
 

WILLOWBROOK CLASS MEMBERS COVID-19 VACCINATION CONSENT INFORMATION 

  • All consents requiring CAB signature should be sent to antonia.ferguson@opwdd.ny.gov
  • It would be appreciated if the agency sent them all together by house or borough, this will also provide one point of contact per house or borough for any questions that may arise.
  • The consents will be addressed by Antonia [Tawnie] Ferguson, Executive Director of the Consumer Advisory Board for CAB and her assistant directors, to ensure timely turnaround.  The goal is for same day or next day depending on the volume received.  The process is anticipated to be completed electronically.
  • The CAB representatives will be working on ensuring that when necessary [i.e. multiple co-morbidities, cancer diagnosis, immunosuppressed, use of epi-pen, etc] primary physicians are contacted and providing approval. 
  • The consents must be completed per DOH/FDA recommendations and should be completed as much as possible to avoid the CAB from tracking down information.

LIFE PLAN DROPDOWN LISTS
In case you missed it, here is the latest Adult Dropdown List and the latest Children's Dropdown List.  Both documents were updated and shared in October 2020 and no significant changes have been posted sense. 


ROLLING OUT LEVEL OF SUPERVISION ADM

Person Centered Services has not rolled out the updated LOS in the Life Plans just yet.  The changes have not yet been made in Medisked.   We will be communicating with all Providers as we educate our Care Coordination team and equip them for the transition, so we can adjust together.

COVID-19 VACCINATIONS FOR THE COMMUNITY

Person Centered Services Care Coordinators are reaching out to those we serve who live in the community and are eligible for the vaccination.  The CCs are providing education and resources on the vaccination, identifying those interested in getting the vaccination and identifying and coordinating any necessary supports for the person to sign up and receive the vaccination.  The level of support and type of support provided by the CC will be unique to each person served.  We encourage all IDT members to be in communication with each other and share updates about vaccinations for people mutually served.
