It would be appreciated if the agency sent them all together by house or borough, this will also provide one point of contact per house or borough for any questions that may arise.
The consents will be addressed by Antonia [Tawnie] Ferguson, Executive Director of the Consumer Advisory Board for CAB and her assistant directors, to ensure timely turnaround. The goal is for same day or next day depending on the volume received. The process is anticipated to be completed electronically.
The CAB representatives will be working on ensuring that when necessary [i.e. multiple co-morbidities, cancer diagnosis, immunosuppressed, use of epi-pen, etc] primary physicians are contacted and providing approval.
The consents must be completed per DOH/FDA recommendations and should be completed as much as possible to avoid the CAB from tracking down information.
